Transforming Care in Provider Practice Champions
At this juncture of national reform, it has becoming increasingly clear that the healthcare environment is changing. Though the details continue to unfold, consistent themes are emerging as essential elements to reform, with a particular emphasis on primary care: coordination of care; quality improvement; and health information technology (HIT). The Transforming Care in Provider Practice Champions initiative, a partnership of the Pittsburgh Regional Health Initiative (PRHI) and Highmark, is designed to introduce primary care providers, practice managers, administrators and frontline staff to key concepts related to practice transformation, along with the tools and techniques to implement change.
The initiative offers training and coaching to support primary care providers involved in the CMS Electronic Health Records demonstration, for which PRHI is the community partner for southwestern Pennsylvania, engaged in the process of practice transformation via health information technology and quality improvement. Supported by a team of coaches from PRHI and Medical Management Consultants from Highmark, providers in the CMS demonstration and the broader primary care community are provided with introductory training and on-site coaching support. The training curriculum is comprised of eight modules, each focusing on a content area relevant to primary care transformation.
