“Excellence in Chronic Care” Forums: A PRHI Series
These forums, free of charge to participants and available for CME/CEU credits to physicians and nurses, aim to:
- Identify, showcase and spread regional successes from outstanding practices in the delivery of care to chronically ill patients;
- Give primary care providers proven tools to change work processes, work organization, and work culture to better align with the Chronic Care Model and evidence-based care.
Target Audience
All staff members of ambulatory care settings, both clinical and administrative
(physicians, nurses, clinical assistants, office managers, office staff,
etc.)
