Community: Resources and Policies

Examples of gaps and weaknesses in our current acute care system

National level
Farm subsidies currently support starchy and sugary crops like corn.

State/Regional
Many public school systems do not offer nutrition information or healthy school lunch choices; availability of candy/soda vending machines on campuses.

Regional/Local
No resources like peer support groups for people with diabetes in the places where they live (workplace, church, community)

No safe neighborhoods for walking and exercising

Limited access to grocery stores or markets with healthy food options

Social services for people who need transportation assistance to go to medical appointments or who need assistance purchasing prescription medications are sometimes available, but rarely well connected to clinician offices or to each other.

 

Examples of implementations of the chronic care model
(these are illustrations and not necessarily prescriptions or endorsements)

National level
Farm subsidies support the growing of affordable healthy fruits and vegetables.

State/Regional
School systems advertise nutrition information and provide healthy school lunch choices; vending machines sell water/juice/fruit, etc.

Regional/Local
Well-advertised and funded programs reach people with chronic illnesses in the places where they live (workplace, church, community)

Neighborhoods have easy access to safe places for walking and exercise

People have easy access to grocery stores or markets with healthy food options

Social services (government and private) for people who need assistance are well connected to clinician offices as partners and to each other—services highly coordinated and accessible.
